“Around ten years ago physicians would often say, “I don’t ever look at my patient’s insurance”, “I don’t know how much things cost”, or “someone else will look into that.” These all conveyed the sentiment of not prioritizing the financial impact to the patient. They were trying to say that they make the same decisions for all patients based on what is best for their diagnosis. While that can still be the intent, there is now a higher level of awareness on the financial impact to the patient. Physicians generally know which insurance plans will cover which type of procedure. By having that knowledge, they are able to have an educated conversation with a patient about what the tentative cost will be to the patient. This can help the physician and patient both to make informed decisions about the plan of care. For example, if a patient needs a procedure but their out-of-pocket cost is $3,000 then the physician might try some additional medication management before going straight to the procedure.
This can get complicated because it can look like the physician is making decisions based off of insurance coverage. It is important that the physician explains that the decision is made off of the best course of treatment while also taking into consideration the cost impact. The intention is for the patient to be as well informed as possible about all elements of care, including cost.
Governance
In large healthcare organizations the governing body is the board, and they help to set the tone for the organization’s goals and priorities. Healthcare has become increasingly quality based both for patient expectations, as well as financial compensation. The board members can help with this quality approach by understanding the elements the hospital is focusing on for quality improvement. They can help to make strategic decisions about which hospital funds go towards supporting those initiatives. For instance, if a hospital has a goal to decrease central line infections, then they might need additional staff to devote time to proper dressing changes or a new type of dressing that has additional antimicrobial properties.
It is also important that the board have a realistic glimpse into healthcare at the organization. Whether they observe patient care situations or talk with physicians and staff, this effort will make an impact. By taking these steps than they can take to make a connection with the team that will help them to be able to make effective and constructive changes.”
