Submit your final report. Be sure to follow the instructions in the syllabus/manual for your final paper. This paper must be typed and uploaded. A final paper/report is required, providing a vehicle for the student to describe and evaluate the placement experience. Paper/Report should be a 5 to 7 page content paper, using the expressed formatting – double-spaced, single-sided, Times New Roman font of 12, and 1-inch margins, with no grammar or spelling errors. Reports should also include a title page, and a table of contents (Not included as the 5 to 7 page content paper). Report sections should include: A discussion of your goals and student learning outcomes negotiated with field placement preceptor and how it relates to your project, A description and discussion of history, philosophy, and agency mission, A description and discussion of the relationship between the health agency and its governing board (a copy of the organizational chart is advisable), A discussion of the organizational climate, and An assessment of your field placement accomplishments. The final report is due the seventh week of the course – No exceptions! The final paper is due the October 12, PLEASE NOTE: The Title Page, Table of Contents Page and Reference Page is NOT included in the the 5 to 7 pages. The 5 to 7 pages should be content only.
