Assignment:
Students will identify a common health concern found among those in a cultural group of the student’s choosing (e.g., chronic hypertension among Filipino American inner city residents).
After describing the impact of this health issue on the population you chose, including statistics on the condition’s incidence and prevalence, students will propose a culturally congruent intervention to prevent this problem. Advanced practice nurses must be able to formulate plans of care that address health promotion and disease prevention activities designed to meet the unique needs of different client populations (AACN, 2011). In addition, these prevention and health promotion activities must incorporate understanding of the health beliefs and practices of this population, incorporating principles of evidence-based and culturally appropriate care.
This is NOT a research paper, YOU are developing, organizing and running a hypothetical community improvement plan for a group of people who you have noticed need some type of intervention. For example, I am an APRN working in a primary care office and I notice that there have been an unusually high number of young, African American males developing renal failure from untreated/unmanaged hypertension. I am going to organize a community fair to screen people for hypertension, give health education re: diet, exercise and weight loss, and refer people for treatment if I find that they have hypertension. You do not have to do the actual intervention program, just write about how you would do it if you were going to implement the program.
This is a hypothetical intervention that you will organize and run. You will provide the details about how you will organize the event.
This paper should:
Describe one specific health issue that affects a specific population in a specific geographic area. This proposed intervention should include statistical data about the extent of the problem in this population as well as details about how the plan would be implemented
Describe the impact of this issue on the selected population
Develop a prevention and health promotion program that could be delivered to this population to decrease (or eliminate) the health concern selected
This proposed intervention should include statistical data about the extent of the problem in this population as well as details about how the plan would be implemented. Details include recruitment of participants, where will it be held, who will fund this intervention, how will you do follow up, what will you teach or what are your interventions and how will you measure if it is successful.
Some ideas for successful completion of this assignment:
First, identify a specific group in a circumscribed geographic area. For example, you may want to target a group from your clinical experience (e.g., persons of Puerto Rican descent in the Philadelphia area).
You could also choose to focus on a group that operates as a “culture” in some other way (for example, the group of seniors in a residential nursing facility or inmates of a specific prison). You will develop an intervention for your specific group, so you will have to take into account the characteristics of the neighborhood or “context,” as well as the culture itself.
Consider information from your readings in identifying problems in specific populations (Chapters from Cupp Curley, in particular).
Your assessment and strategy must take into account the health education, health literacy and health promotion needs of your target population.

Specifics:
This is a formal paper and should be written as such with abstract and references, in APA format. Following APA format, there should be a title page, a running head, pagination numbers, and an abstract. Include your PICOT question after the abstract paragraph.
Scholarly writing does not employ use of the first person.
The final paper should be no more than 4-6 pages, not including title/reference page.
Students are to submit the paper via Canvas by the assigned date.

Document your descriiption of the extent and impact of the problem WITH RESEARCH AND FACTS from PROFESSIONAL sources. This is an evidence-based assignment, so make sure your evidence is current (within the last 5 years) and from peer reviewed resources in the literature.
Paint a clear picture of the health concern in your population. For example, here are some questions to consider:
What evidence-based RESEARCH has been done evaluating delivery of services to your population? For other groups?
What are the standards of best practice in regard to your selected health concern? (See professional literature/search Cochrane database)
How will you evaluate the effectiveness of your program (measurable, observable, relevant outcomes) from your program? What additional data would you like to have had in describing either the health concern or the proposed intervention?

Solutions should specify how the advanced practice nurse will engage in the development and delivery of a client-centered and culturally responsive program to address this problem. You are running this program, how would you set it up, recruit participants, get a meeting place, keep track of changes, get funding and measure outcomes.
