Instructions for the Bio-Psycho-Social-Spiritual/Plan of Care Assignment Rough Draft
You will need to submit a rough draft here of the scriipt that you plan to use when you complete your final Bio-Psycho-Social-Spiritual/Plan of Care. Remember, this is to be an original scriipt of questions/areas you plan to address in your assessment with your ‘client’, it is to be an original document created and written by you. You can use resources from the text or online platforms to get ideas, but make sure not to plagiarize (cannot copy directly from another source). Use your critical thinking skills to develop your own assessment. In addition to your scriipt of questions for your Bio-Psycho-Social-Spiritual Assessment, you will need to submit a rough draft here of your Client Contract and Plan of Care.
Important Components to Include:
Assessment questions must be made of the client at all system levels to include: the Individual, Family, Group, Organization, and Community.
The client contract will clearly state the problem, goals, and objectives; it is essential here that you also provided information about informed consent and limits of confidentiality.
The plan of care must address action steps at all system levels: (Individual interventions, Family interventions, Group interventions, Organizational interventions, and Community interventions).
You can submit as one document just make sure to use headings that clearly separate each of the 3 components (Assessment Questions; Client Contract; and Plan of Care).
So this is a 3 part writing assignment both for the draft and the final. As mentioned in details of the assignment (under Projects and Papers Module), you will need:
1. Bio/Psycho/Social/Spiritual Assessment: questions must be made of the client at all system levels to include: the Individual, Family, Group, Organization, and Community.
2. Client contract: clearly state the problem, goals, and objectives; it is essential here that you also provided information about informed consent and limits of confidentiality.
3. Plan of care: address action steps at all system levels: (Individual interventions, Family interventions, Group interventions, Organizational interventions, and Community interventions).
You do not have to write a full scriipt that includes what the answers from the ‘client’ that you plan to interview and video record have to be; your client’s responses can be honest and unscriipted; but you need to go ahead and discuss ahead of time what issue the client will be coming in to address so that you can create your client contract and plan of care draft. What I am looking for as ‘scriipt’ is the assessment questions you intend to use to make sure you cover all the areas needed to complete a Bio/Psycho/Social/Spiritual Assessment. Biological – basic questions about medical conditions, medications, diet, exercise, etc. Psychological – questions about any mental health diagnosis or concerns, emotional state and areas of concern, substance abuse, trauma, etc. Social – questions about their support system, family dynamics, hobbies/clubs, what they do for work and fun, meaningful places and people, etc. Spiritual – hold any spiritual beliefs or observe certain practices, belief in a higher power of any sort, coping skills associated with spirituality, etc.
Client contract that provides information about informed consent and limits of confidentiality and will clearly state the problem (what they are here to get help with), goals (outcome the client wishes to achieve by working with you — end big picture), and objectives (specific and measurable action steps that will help them reach their goal — practical steps to take to make the big picture a reality).
Example: (Problem) Client comes in today to receive assistance with struggling to obtain employment. (Goal) Clients wishes to obtain a job within the next 3 months. (Objective) SW will work with client to improve and update resume within 1 week. SW will work with client to secure and complete three job applications every week for the next 3 weeks. SW will work with client to secure at least one job interview within the next 4 weeks.
Plan of care that address action steps at all system levels: (Individual interventions, Family interventions, Group interventions, Organizational interventions, and Community interventions). So here is where you would use strengths from your Bio/psycho/social/spiritual assessments to incorporate other strengths and resources that client could pull from in their life to help overcome the current problem. Obviously for the draft before you actual conduct the assessment you may not know details here but you can go ahead and determine what you would be looking for in your assessment.
For example: (Individual) SW will work to identify achievable positive steps that client can take to obtain a job, such as being responsible for finding at least 3 job postings a week he is interested in and/or could be capable of doing. (Family) SW will work with client to identify a person in the client’s family that may be of a resource in fine tuning or learning a skill set that would be marketable to those jobs that are available. (Group) SW will work with client to identify social circles he/she is already involved in that may offer job opportunities. (Organization) SW will work with client to identify organizations that may offer assistance in overcoming barriers to employment such as transportation or childcare. (Community) SW will work with client to identify community programs that match with the client’s means and values that may offer assistance in job training or education.
