Your patient describes the headaches as a dull ache generalized throughout with a lack of interest in usual activities. Symptoms began approximately three weeks ago, and progressively been getting worse. The past medical history includes depression and psychosis (diagnosed at age 23). When asked about alcohol intake and current prescriptions, it was reported three red-bull with vodka’s are consumed every night, smokes ½ a pack of cigarettes per day, and admits to not being compliant with medications. You note some concerning behaviors including delusions, disassociation, and difficulty focusing on the conversation. Your patient also admits to feeling sad almost all the time with thoughts of hopelessness and worthlessness (denies suicidal ideation).
Current prescriptions
Fluphenazine 30 mg PO every day
Venlafaxine 225 mg PO every day
Tylenol (Acetaminophen) 650 mg PRN for headaches
