You are the Director of Human Resources for a plant in Gainesville, GA. You are responsible for all communication with OSHA. Four days ago, three injuries occurred in the plant. Use the OSHA forms included in the module and complete them based on the following incidents:
1. Machine Operator smashed his thumb in a machine. First Aid was provided on the plant floor. The machine operator went home early for the day. He returned to work the next morning for his regular shift.
2. An office manager slipped while she walking down the stairs at the plant. As a result of the fall, she broke her arm. She received medical attention at the hospital and will return to work 1 week from the date of the incident.
3. An area of the plant was not properly ventilated as required by policy. A worker in the area passed out while working and bumped his head on a stool while falling to the ground. The worker was transported to the hospital by ambulance. Medical staff were able to treat the patient and decided to monitor him overnight. The worker was released from the hospital the next day and received documents from the doctor stating that he could return to work i 1 week from the date of being released from the hospital.
