List the signs and symptoms that
point to a mental health diagnosis in the case.
Describe the other case details you
would need to know for treatment planning and explain your reasoning.
List and describe the treatment
interventions you would use for this case and explain your reasoning.
Instructions for answering the
Case Study
In this Case Studies, you
are the therapist and the person in the case study is your client. When
writing your response for each section, remember to put yourself in the
shoes of the therapist. As the
therapist, you have the skills and education to provide therapeutic care for
your client.
Your client’s diagnosis is written
within the case study. You must read the
entire case study and you will see the diagnosis. Please write it exactly how it is written
within the case study (do not shorten it or add to it).
You must use the name that
is given to you within the case study.
If there are two case studies, you
must do both. If there is one, then you
must do the one.
The only time you will refer your
client for outside services is when they are in need of the following:
v
Referral to psychiatrist for medication
evaluation to assess need for medication.
v
Referral to physician to rule out medical issues
or to address medical needs.
v
Referral to nutritionist for an evaluation to
address dietary needs.
v
Referral to neurologist for an evaluation to
rule out neurological needs or address neurological needs.
As a therapist, we sometimes
coordinate care with other providers to assist our client.
As a therapist, we also provide
psychotherapy to educate our client and family members about their illness,
treatment options, and resources.
As a therapist, it is not unusual
for us to provide both individual therapy to our client and at times family
therapy with our client and their family members to help support our client’s
needs within the family system. However,
if our client is in need of marital counseling, it is best to refer them to a
marriage counselor while we continue to provide individual services to our
client. This can help us avoid
unforeseen ethical dilemmas.
When completing the
Case Study Form, keep in mind the following:
You are the therapist and the case study is your client.
Do not refer your client to a therapist or refer out for therapy.
Diagnosis – you must use the
diagnosis that is written within the case study narrative.
Symptoms section – List
all signs and symptoms for the mental health condition reported within
the case study narrative (feel free to write
using bullets to make it quick and easy to document).
Details Need to Know section
– explain what else you would need to know that was not shared with you that
can make a difference in treating your client.
Look at the example and format within the case study example. Please do not use the word “I”.
Treatment Interventions section
– see what your textbook recommends for treating this
type of disorder and use it as your guide. Choose treatment interventions that are
best suited for your client. You
must state each of your interventions and write the purpose next to
each of them. It must fit the needs of your client and it must be
written for your client. Do not
use the word “I”. Please refer to the case study example for
guidance. Intervention
Example: Solution-focused therapy – assist client with exploring
problems and possible solutions.
