Based on a preliminary health assessment and behavioral change contract, students will choose something about their current lifestyle or health pattern to change or improve.
You decided on a Behavior Change at the beginning of the semester. You will provide responses based on that behavior change.
Students will utilize the outline below to create a behavior change project.
You should have chosen an improvement that will be easy to measure.
Utilizing the outline below for your Behavior Change, you should have begun your project by the second week. Some of you did not have journal entries at midterm, do not try to make them up at this point. Use the entries that you have made since then if any.
Establish a health goal(s). What is it that you plan to work on or change? (e.g. to lose weight, watch less television, etc.)
Establish measurable objectives. How do you plan to achieve your goal(s)? (e.g. I will eliminate one soft drink per day for one week and replace it with 10 ounces of water.)
Determined baseline condition. How were you in this area at the start of the project? Write in measurable terms such as “I currently drink 12 cans of soda a week when I am at work.”
State the action to be taken. (e.g. Increase water consumption from 2 glasses a day to 8 glasses a day over the next 14 weeks.)
Provide measures of success and relevant documentation. Using a journal, pre-post measures, supporter sign-off, a calendar, etc.
Discuss barriers to implementation. Share expected and unexpected barriers. In this areas, things could be related to your environments or lifestyle.
Discuss supports. (e.g. Keeping a low fat snack bag in the car to eat instead of stopping for fast food.)
Analyze various ways that you can weaken the barriers and strengthen the supports.
Tracking Tool (Journal Entry Template): You were required to track behavior on a regular basis. The journal entry template that you used and submitted at various points through the course must be submitted as an appendix at the end of your final paper (copy and paste the entire document into the end of your paper, after your reference page).
