Choose an individual to assess (does not have to be a patient, cannot be yourself, but can be a family member, friend, co-worker, etc.).

Perform nutritional history.
Perform physical assessment related to nutritional assessment. If available include any laboratory data relevant to the nutritional assessment.
Thoroughly document nutritional findings.
Interpret the assessment data and prepare 2 nutritional outcomes or goals. Outcomes or goals should be measurable, attainable, specific, and patient-centered.
Include at least one intervention for each of your outcomes or goals.
Support your interpretation of the findings, outcomes or goals, and interventions with at least 2 scholarly resources.
APA 7th edition format required. Resources should be no older than 2016.
Submission Guidelines
After filling out the Assessment table utilize the information and write a 3-5 page (not including the title and references pages) paper using essay format. Only doc, Docx, and odt file types are accepted – PDF files are not accepted. You may download Microsoft Office for free under Pages.

Review the assessment table document to prepare and familiarize yourself with the data to be collected **Note: Please disregard the reference to “page 9” of the text for the section on Dietary Data.
Conduct interview and take notes using the assessment table.
Write up 3-4 page paper with findings from the nutritional assignment you conducted, along with the analysis for your patient care plan. **Please refer to the grading rubric to make sure you have addressed all required components of the assignment.
