Purpose. What is the purpose for which this assessment/measure/survey/instrument will be used?

Construct Close-ended Items. This must be formatted as an actual measurement instrument that can be distributed to participants. Please construct 3 demographic questions (age, gender, income, etc.) and 5 questions dealing with a topic of your choice. These five may be knowledge assessments (multiple choice), opinion questions (Likert response scale), or other close ended types of questions.
Construct Open-ended Item. Write one question that requires respondents to give a short text answer about two or three sentences long.
Format. The design of the survey matters. There should be plenty of white space, questions should be clear and concise, and there should be instructions to the user about how to complete the instrument.
