Instructions to Students

Purpose:  Learn how to construct a Fishbone diagram

Scenario:  Late Pizza Deliveries

1. Brainstorm why pizza deliveries are late sometimes
2. Organize your brainstormed ideas into the following categories:
   1. Machinery/Equipment/Plant
   2. People/Employees
   3. Methods/Policies/Procedures
   4. Materials/Supplies
   5. You may use “Other” category if necessary
3. Construct Fishbone Diagram utilizing the template (you may use Word, Excel, or Power Point):

5. Submit under Assignment.

Instructions to Student

Purpose:  Learn how to construct a Pareto Chart

Scenario: Late Pizza Deliveries Reasons and the Need to Reduce Late Pizza Deliveries going forward

1. Assume that the total number of late pizzas in the past year equals 1,000
2. Given your  brainstormed reasons in Part A, assign a value to each reason (e.g. oven was not pre-heated on time = 150 times out of 1,000)
3. Construct Pareto Chart utilizing the following template (excel is the best tool to use to construct the Pareto chart):
   • Please note that “Category D, A, B, E, C” below have to correspond to your reasons for late pizzas deliveries.

Do not forget the cumulative percentage line.

1. You do not have the budget to address and eliminate all reasons for late pizza deliveries. You need to recommend the actions to take based on the constructed Pareto Chart with the detailed explanation.
2. Submit under Assignment.
3. Please see the attached Fishbone and Pareto diagramming free online tools.
4. Submit your work utilizing one of the free online Fishbone and Pareto tools provided or any other free tools that you are aware of. There is no penalty for using different tools that provided by an instructor.
