For this assignment, you will choose a family member or friend to be your practice “client” and implement some of the key processes of case management with them this semester.

Part 3: Coordinate services with community resources to help your client and use proper documentation through at least three progress notes. Discharge your client either through referral or successful discharge and end services using the skills learned in class.
