Learning Goal: I’m working on a health & medical discussion question and need an explanation and answer to help me learn.I need a soap note for a patient that you can make up. This patient should be seen in an outpatient clinic by a PCP (example hypertension, pain, fractured bones ect. nothing serious or life threatening). No diabetes appointment for these two patients. The visit is a write up of their visit to the outpatient clinic. The paper does not need to be in any format like APA. The patient has to be between the ages of 21-65 and can be male or female. Medication and labs need to be detailed as far as levels, doge, amount of times taking a day. Also I will attach two samples that were provide on what our paper needs to look like.
