To be clear, a narrative summary of all steps should be included in the body of the paper in order to provide a description of the process to the public and the completed Logic Model Template, 90 – 180 Day Implementation Worksheet, Developing an Evaluation Plan, and C4HEvaluation Plan Template documents should be included in their own respective appendix. Examples of each of the worksheets are provided and in the attached documents for your assistance/reference.
