You will analyze the performance of the crew (and other teammates such as ATC, fire/rescue, dispatch, maintenance) with respect to this module’s lesson on leadership and teamwork. Highlight both positive characteristics and areas for improvement. In your analysis, include a discussion of the principles of 6th Generation CRM/Threat and Error Management (even though this accident was pre-6th Gen.). Do not just summarize the events; include a critical analysis!

The Communications Loop (see diagram) is a critical part of every single flight. It is virtually impossible to engage in any flight activity, from general aviation to military tactical flying, without the use of some degree of communication. This communication might be between the pilot and other crew members, ATC, dispatch, Flight Service, or even other aircraft. Methods of communication might include verbal, body language (such as hand gestures), written, or data link/CPDLC. However, despite how critical communication is, research has shown that pilots and controllers make MANY communication errors. Why is this?
For this discussion, you will consider common causes for breakdowns in the Communication Loop. Determine the meaning of “Noise” and “Filters” in the Communication Loop diagram. List at least five (5) common filters or barriers to communication. You may use examples from your own experiences. Then recommend methods to overcome filters in communication. Is one of the listed methods of communication less susceptible to noise, filters, and errors? If so, which one, and why?

Assess the impact programs such as LOSA, ASAP, and ASRS have had on aviation safety. Why were these programs started, and what did they hope to achieve? In your discussion, analyze the benefits of both direct observation of aircrew (LOSA) and anonymous self-reporting systems (ASAP/ASRS). Determine if there are any potential negative aspects of such programs and why participation was slow to catch on in some segments of aviation. How do these programs relate to airline CRM programs?

Over the course we have looked at the history, skills, and training skills of CRM, but how do you know if CRM is part of an organizations culture? How do you evaluate CRM? The cultural change of flying did not happen overnight. How long do you think it takes to change a culture in aviation? Before the 1970s the First Officer was graded his ability to sit in the seat and not say anything to the Captain, even if he/she was making mistake. Today, even the flight attendants are encouraged to speak up if something doesn’t feel right. Example: Northwest Airlines Flight 188 landed at Minneapolis over an hour late because they claimed to be distracted by their iPads. Watch the entire YouTube video, Remembering Flight 232: Short Story of Disaster & Survival hosted by J. J. Stone. The United Airlines 232 flight is a little bit different when analyzing CRM. The flight still ended tragically with 111 people killed, but could have or should have killed everyone on board if not for amazing CRM. When watching the video notice how CRM was just not with the crew that started, but also included the DC-10 instructor that was flying as a passenger.
