Community nursing is a synthesis of nursing practice and public health practice that is applied to improve and maintain the health of the population. “Public health nursing (PHN) practice is population-focused and requires unique knowledge, competencies, and skills. Early public health nursing roles extended beyond sick care to encompass advocacy, community organizing, health education, and political and social reform (Kulbok, 2018)”. The targets of community health nursing are individuals, namely malnourished toddlers, high-risk pregnant women, the elderly, people with infectious diseases. The target family is families who are vulnerable to health problems and priorities. Target specific groups, both healthy and sick communities who have health or treatment problems.
Community Nursing is a professional nursing service aimed at the community with an approach to high risk groups, in an effort to achieve optimal health degrees through disease prevention and health improvement by ensuring the affordability of health services needed and involving clients as partners in planning, implementing, and evaluating services. nursing. “The nurse providing guided care offers eight services: assessment; planning care; monitoring; coaching; chronic disease self-management; educating and supporting caregivers; coordinating transitions between providers and sites of care; and facilitating access to community services, such as Meals-on-Wheels, transportation services, and senior centers. Results of a pilot study comparing surveys of patients who received guided care and those who received usual care revealed improved quality of care and lower health care costs (according to insurance claims) for guided care patients (NIH, 2019)”. Community Nursing Services are the entire community including individuals, families and high-risk groups such as families living in slum areas, isolated areas and inaccessible areas including groups of infants, toddlers, the elderly and pregnant women.
The community nursing process is a natural, systematic, dynamic, continuous and continuous nursing care method in order to solve the health problems of clients, families, groups and communities through steps such as assessment, planning, implementation, and evaluation of nursing.
Community health nurses can work with communities and populations to reduce health risks and promote, maintain and restore health. Community health nurses advocate at the system level for health change. Community health nurses must understand and apply concepts from a variety of areas. Community nurses must also apply the concepts of community organizing and development, care coordination, health education, environmental health and public health sciences.
